• Affidavit of Student (Original)
• Passport (Original)
• Insurance must be bought from here.
• Confirmed Air Ticket to Poland
• All Academic Documents Original and as per following :
  • 10th mark sheet & certificate (Photocopy with True copy & Notary )
  • 12th mark sheet & certificate(Photocopy with True copy & Notary )
  • Graduation mark sheet &Degree certificate (Photocopy with True copy & Notary) (Bachelor - Provisional Degree) (Photocopy with True copy & Notary )
• Apostile of Last Education Required (Compulsory)
• IELTS certificate if applicable
• Marriage certificate (if applicable)
• Marriage certificate (if applicable)–(Photocopy with True copy & Notary)
• Work experience (if applicable than Original required)
• Affidavit of Sponsor (Original)

Financial Docs :-

• Education Loan
• Valuation Report
• Bank Statement/ Bank Passbook of last 6 months (Balance must be 3 months old)
• Income Tax Returns Last 3 Years
• F.D. certificate (Must be 3 months old)
• PF / PPF / GPF (withdraw able amount) statement

Family Details

• 1. Parent (Father and Mother ) /Legal guardian’s detail :
  i. Name
  ii. Date of birth
  iii. Place of birth
  iv. Passport Detail (passport no, expiry date)(If have)
  v. Guardian (Father & Mother ) – Occupation
• 2. Full list of all Family Members (brothers, sisters, spouse and children, including deceased or adopted members)
  i. Name
  ii. Date of birth & Place of Birth
  iii. Relationship to applicant
  iv. Passport Detail (passport no, expiry date) (If have)
  v. Family members Occupation

If you have ever been refused a visa for any other country, details of this, preferably the original letter issued to you by the authorities of that country, should be submitted.
